Appointments:

We require a 24 hour notice of cancellation. There will be a $25.00 charge.

In order to make payment plans convenient for our patients, we offer several options for payment.

Insurance:

Your insurance is a contract between you, your employer and the insurance company. We are not a party to the contract. We file to your insurance company at no charge to you as a courtesy. We are more than willing to provide information to the insurance company to allow maximum benefit for your policy. However, it is your responsibility to know what those guidelines are and let our receptionist and the doctors know. Unfortunately, since insurance plans are payment assistance plans, dental benefits may be subject to review and is stated to never be a guarantee of payment. If you do not inform us of any special requirements in your policy and subsequently lab work, x-rays, and/or other treatment is ordered that the insurance company may not cover, we will have no choice but to bill you directly. PAYMENT FOR THESE CHARGES WILL BE YOUR RESPONSIBILITY.

We accept most insurance; including payments from PPO plans. Please feel free to call our office with any questions or voice any concerns you may have. Patient Co-Pay is due at time of service.

Payment Options:

Pay for services when rendered. We accept all major credit cards, ATM cards, cash, and personal checks. We offer the option of Care Credit financing. Please ask the receptionist for brochure.

There is a $30.00 charge on returned checks. Any balances over 30 days are subject to finance charges. Patient or responsible party agrees to be responsible for the costs of collection of any unpaid balances plus interest over 30 days, including collection agency fees.
